ALL ATHLETES WANTING TO PARTICIPATE IN SUMMER ATHLETICS AND '25-'26 SCHOOL YEAR MUST HAVE A NEW PHYSICAL ON FILE. NEEDS TO BE HANDED TO THE ATHLETIC TRAINING STAFF.
Physicals must be dated AFTER April 15, 2025

- Must include answers to "yes" questions at the bottom of the page. ALL boxes must be completed and signed.
- Physical
- Must include clearance, date, and signature.
- We will NOT accept signatures from doctors of chiropractic

- TWO forms completed online
- Name must be the same as the one you use to register for school (ie. multiple last names, hypen between them, etc.)
- Click "continue as guest" and fill out both forms. These must be completed EVERY academic year
- Do NOT print out additional forms
