Annamarie (Annie) Sciacca, RN, BSN, PHN - Campus Nurse
Graduated from The Valley Foundation School of Nursing at SJSU with Bachelor's of Science in Nursing and PHN in 2014. Completed Credential in School Nurse Services at CA State University Fresno in 2019. 12 years working in CA and TX as an RN, working in Psychiatric Health, Children’s Health, Disease Prevention, and as a School Nurse (credentialed in CA). 3 years as the Akins ECHS nurse, some time at Lively MS and Paredes MS and one year at Travis ECHS.

Suzanne (Samantha) Maldonado - Health Room Assistant
Has worked within Ascension/AISD for 18 years. May see students in the health room and administer appropriate medications. Trained as an unlicensed diabetic care associate and certified in First Aid/CPR.

Please complete a consent form if you would like your student to receive non-emergent care for the 25-26 school year. You can opt in and out of several nursing services.

To attend school, Texas law requires official immunization record, medical exemption or affidavit on file:
- Bring an updated immunization record after receiving necessary vaccines.
- Reach out to the nurse, via email, for a list of low cost clinics in the area.

If your child has a medical condition that requires care at school such as (but not limited to) Seizures, Asthma, Anaphylaxis/Severe Allergic Reactions, Diabetes:
- Schedule an appointment with me to go over provider orders and meds
- Bring a recent copy of the doctor’s order, school time instructions, or action plan
- Bring a completed Provider/Parent Medication Permission Form (physician and parent signature)
