Required Forms for Athletic Participation:

AISD Physical Form 2026-2027 (English)

AISD Physical Form 2026-2027 (Spanish)

**Physicals must be done for each academic year AFTER April 15th. (For the 2026-2027 school year, physicals must be dated after 4/15/26 - and physicals dated prior will be invalid).
